Overview:

This session provides a focused review of functional status Quality Measures with emphasis on ADL coding accuracy and its impact on quality outcomes, Five-Star ratings, and reimbursement. Participants will examine how functional decline and improvement measures are calculated, common MDS coding pitfalls, and the role of interdisciplinary documentation in supporting accurate reporting. Practical strategies will be shared to help facilities strengthen functional outcomes while maintaining compliance.

Objectives:

At the end of this session, participants will be able to:

  1. Describe how functional status Quality Measures are defined and calculated
  2. Identify common ADL coding and documentation errors
  3. Explain the relationship between functional outcomes, Five-Star ratings, and reimbursement
  4. Apply practical strategies to improve functional QM performance

Sarah’s expertise spans Quality/QAPI program development, federal and state reimbursement strategies, MDS accuracy, performance improvement, clinical education, and healthcare coaching. Known for her collaborative approach, she empowers healthcare teams to optimize clinical outcomes while navigating complex regulatory environments.
Sarah is certified by AAPACN as a Director of Nursing Services-CT (DNS-CT), Resident Assessment Coordinator-CT (RAC-CT), and Quality Assurance Performance Improvement Certified Professional (QCP-CT). Her passion lies in guiding organizations to achieve clinical excellence and maximize quality/reimbursement outcomes.
