Discharge Planning Compliance with CMS Acute and CAH CoPs

This program will cover the requirements for discharge planning. The regulations apply to all hospitals, including Critical Access Hospitals, with the goal of reducing preventable readmissions. This program will discuss the Impact Act and how it affects hospital discharge planning. The regulation requires standardized assessment, quality data, and resource data requirements. The program will address what is required of a hospital to assist patients with post-discharge care plus timely access to medical records for decision making processes to occur.
